Summary:
- The team will work closely with Adult Mental Health inpatient services to jointly plan safe , supportive and timely discharge for people from hospital.
- The team will ensure that all people with complex mental health needs in receipt of support, particularly those to whom the Borough of Sutton has a duty under S117 of the Mental Health Act (Aftercare) are identified and offered expert mental health social care assessment and support planning.
- Building strong relationships with partners in the statutory and third sector will be an important area of work for this team.
Requirements of the Team Manager:
- Degree or equivalent in social work
- Current Social Work England registration
- Recent managerial experience, working as a TM, DTM or ATM etc.
- Significant frontline experience in Adults Services
