Our team is dedicated to supporting older adults and individuals facing the challenges of physical, cognitive, or learning disabilities within the community. We are committed to a person-centered approach, empowering individuals to live independently while ensuring their safety and well-being.Responsibilities:Manage assigned cases, implementing legal and procedural measures, including safeguarding principles, to protect vulnerable individuals from abuse or neglect.
Utilize a strengths-based, person-centered approach to support individuals in developing and maintaining their capacity for independent living and positive risk-taking, in accordance with the Care Act principles.
Conduct Care Act assessments, support planning, Mental Capacity Assessments, and make best interest decisions.
Demonstrate understanding of Deprivation of Liberty Safeguards (DoLS) and work within the legal framework, including experience with s.21a court work.
Adhere to safeguarding policies and procedures, conducting Section 42 enquiries as necessary.
Complete Continuing Health Care Checklist and Decision Support Tools as required.
Undertake home visits with customers as needed, traveling independently throughout the Rotherham Borough.
Requirements:Experience working with older adults and/or individuals with physical, cognitive, or learning disabilities in community-based or locality-based adult social worker teams.
Proficiency in Care Act assessments, support planning, Mental Capacity Assessments, and making best interest decisions.
Understanding of Deprivation of Liberty Safeguards (DoLS) and experience with s.21a court work.
Familiarity with safeguarding policies and procedures, including conducting Section 42 enquiries.
Ability to work independently and travel independently to and around the Rotherham Borough.Benefits:Competitive pay rate of £32.99 per hour.
