Job Summary:

Reports to the Virtual Health Manager, the Remote Patient Monitoring RN is primarily responsible for managing patients that are enrolled into the Remote Patient Monitoring (RPM) Program. The Remote Patient Monitoring RN is responsible for the medical triage of the vital signs using established protocols and providing specific guidance per said protocol using clinical judgment. The Remote Patient Monitoring RN is also responsible for scheduling appointments for patients as appropriate, collaborating with the overseeing provider, educating patients on chronic disease management, and encouraging program compliance. The Remote Patient Monitoring RN is a member of the Virtual Health Team and collaborates with providers across the system to provide safe, timely, effective, and efficient patient care.

POSITION DESCRIPTION

Job Title: Remote Patient Monitoring RN 

Non-Exempt 

MUST LIVE WITHIN 1 HOUR OF ERLANGER KY TO BE CONSIDERED

This is a 30 hour/ week position, working 10 hours on Monday- Wednesday-Friday each week. 

PRIMARY PURPOSE:

The St. Elizabeth Physicians (SEP) Remote Patient Monitoring RN is primarily responsible for managing patients that are enrolled into the Remote Patient Monitoring (RPM) Program. The Remote Patient Monitoring RN is responsible for the medical triage of the vital signs using established protocols and providing specific guidance per said protocol using clinical judgment. The Remote Patient Monitoring RN is also responsible for scheduling appointments for patients as appropriate, collaborating with the overseeing provider, educating patients on chronic disease management, and encouraging program compliance. The Remote Patient Monitoring RN is a member of the Virtual Health Team and collaborates with providers across the system to provide safe, timely, effective, and efficient patient care.

DUTIES AND RESPONSIBILITIES:

  • Understands and upholds SEPs Mission, Vision, and Values.
  • Comply with all applicable laws and regulations.
  • Triage incoming patient vital signs such as, but not limited to blood pressure, heart rate, weight, temperature, oxygen saturation and ensure clinically appropriate care is delivered.
  • Schedules appointments for patients. Uses manual/computerized system to match provider availability with patients preference in terms of date and time.
  • Communicates as needed with providers and other staff about any patient concerns/issues related to scheduling.
  • Ensures complete and accurate information in the Electronic Health Record
  • Assists patients with healthcare related technology needs, including but not limited to: RPM Device(s) Video Visit Connections
  • MyChart set-up
  • Communicates in an effective and professional manner with patients, providers, support employees, co-workers, and management.
  • Contributes to the development of protocols, procedures, patient education, and training as assigned. Participates in peer review, quality assurance, and other committee functions.
  • Other duties and responsibilities as assigned.

EDUCATION:

Minimum: Graduate of accredited Registered Nurse Program

Desired: Bachelors of Science in Nursing (BSN)

YEARS OF EXPERIENCE:

Minimum: 3 years of RN experience

Desired: Experience in chronic disease management, experience in both inpatient and outpatient setting

REQUIRED SKILLS AND KNOWLEDGE:

Ability to manage and prioritize multiple tasks. Knowledge of Electronic Health Records (EHR) EPIC. Knowledge of Microsoft Excel, Word, Outlook and PowerPoint and the ability to learn other computer skills as needed. Work professionally with providers, administration (SEP and SEH), associates and the public. Organized, neat and self-motivated. Warm personality and concern for others. Excellent verbal and written communication skills. Excellent interpersonal skills. Ability to affect change. Ability to perform critical analysis. Self-directed. Work well telephonically as well as face-to-face. Ability to work autonomously. Positive attitude. Quest for learning and excellence.

LICENSES AND CERTIFICATIONS:

Minimum

  • Must have a valid drivers license for travel between office sites
  • Kentucky Registered Nurse (RN) license required to be hired
  • Must obtain Ohio and Indiana Registered Nurse (RN) licenses within 30 days of hire
