Meath) Greenarces (Clonard, Co. Meath) The Cottage (Enfield, Co. Meath) Bay House (Navan, Co. Meath) Moyne House (Duleek, Co. Meath) Oaklands (Navan, Co. Meath) Aingeal Lodge (Lobinstown, Co. Meath) Chestnut Grove (Navan, Co. Meath) Kilbride House (Bray, Co. Wicklow) The Coach House (Balbriggan, Co. Dublin) Greenfields House (Newbridge, Co. Kildare) Carraig Lodge (Carrickmacross, Co. Monaghan) Teach Beag (Carrickmacross, Co. Monaghan) Cois Cuain (Rosslare Harbour, Co. Wexford) Darby’s View (Stradbally, Co. Laois) We provide residential support services for adults and children with intellectual disabilities, autism spectrum disorder and acquired brain injuries. As a Health Care Assistant/Direct Support Worker, you will work as part of a multi-disciplinary (MDT) team to support our residents to live happily and independently in their new home. You will provide care and support through a person-centred approach which promotes residents’ development, social inclusion and activity in their local community. Your duties will be guided by your Team Leader/Person-In-Charge (PIC) and include, but not be limited to the following; Deliver on residents’ daily support plans and ensure ongoing focus on their strengths Monitor the health and wellbeing of residents and report concerns to your PIC/Nurse. Assist and promote good personal care skills i.e. bathing, toileting, showering, dressing, ensuring residents’ rights to dignity, respect and privacy are maintained at all times. Support residents to manage their personal medication in line with current practices, where applicable. Accompany residents to outside medical and clinical appointments, where applicable. Document and relay information to relevant people as required i.e. families, MDT team Carry out any duties necessary to ensure the comfort and hygiene of residents. Support residents to work towards achieving their personal goals, guided by their individual care plan. Act as a key worker for a specific resident where appropriate. Encourage and support residents to take an active part in the community. Perform additional duties from time to time as directed by management. A relevant Healthcare or Social Care qualification, minimum QQI Level 5 (e.g. degree/certificate in healthcare, nursing, midwifery, allied health, medical, etc.) and/or Relevant experience in healthcare or a care environment
