HSE Mid-West are currently recruiting for the role of Clinical Nurse Manager 2 – Discharge LoungeLocation of the PostHSE Mid-West Acute Services – University Hospital LimerickThere is currently one permanent, whole-time vacancy available in the Peri-Op Directorate, University Hospital Limerick.A panel may be formed as a result of this campaign for Clinical Nurse Manager 2 – Discharge Lounge, HSE Mid West Acute Services from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled.Purpose of the PostThe Clinical Nurse Manager 2 – Discharge Lounge will· Ensure that patients in the Discharge Lounge are discharged in a safe and appropriate manner and have all the relevant discharge documentation on leaving the hospital.· Ensure Discharge Lounge Log is completed.· Check/Organise that patient’s transport arrangements are in place.· Ensure that discharge documentation has been completed – discharge letter, prescription etc.· Ensure that patients do not leave the Discharge Lounge without relevant documentation and nursing discharge checklist documentation being completed.· Ensure doctors are aware that they must arrange follow up, other than an out-patient’s appointment if appropriate.· Contact admissions to complete the discharge from iPMS in a timely manner.· Ensure that patient’s healthcare record is delivered back to the inpatient ward where the patient was transferred from.The post of CNM 2 has a pivotal role in service planning, co-ordinating, and managing activity and resources within the clinical area.
