HSE Mid-West is hiring Candidate Advanced Nurse Practitioner (cANP) Venous Thromboembolism (VTE).Location of PostHSE Mid-West There is currently 1 permanent / whole-time vacancy available in University Hospital Limerick. A panel may be formed as a result of this campaign for Candidate Advanced Nurse Practitioner (cANP) Venous Thromboembolism (VTE) from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled.Purpose of the PostTo contribute to the continuous improvement of the quality of care experienced by patients with VTE and related issues to diagnosis (i.e. thrombosis, haemostasis and anticoagulant-related issues).  The specific contribution of the ANP is to improve outcomes for patients who fall within the ANP scope of practice.

  • The ANP will compliment and contribute to the overall service provided by the Thromobsis Service.
  • The ANP will contribute towards the department’s aim of making University Limerick Hospital Group a centre of excellence for the management of patients with venous thromboembolism (VTE).
  • To take the lead in developing an Ambulatory DVT Pathway, with the purpose of improving the patient experience, patent safety, reducing emergency department wait times, reducing referrals to the medical on call teams and reducing medical outpatient waiting lists.
  • To develop and oversee a safe pathway for the early discharge and community management of patients with low-risk pulmonary emboli, thus reducing the number inpatient hospital bed days.
  • To ensure that patients with acute VTE events have early follow-up postdischarge. This is in accordance with international best practice, and serves the dual purposes of patient education and identifying anticoagulation-related issues early.
  • In line with University Limerick Hospital Group position as a designated cancer centre and the fact that thrombosis is the second leading cause of death in cancer patients, the VTE ANP would be expected to develop expertise in cancer associated thrombosis (CAT), and co-ordinate a multi-disciplinary team meeting for CAT cases
  • To assist in the reduction of University Limerick Hospital Group outpatient waiting lists relating to VTE patients.
  • To develop the interface between primary and secondary care services (General Practice, External Hospitals, PHNs, and Practice Nurses) through clinical work, education, research and leadership.
  • To take the lead in developing University Limerick Hospital Group education programmes for clinical staff, patients, and their families in, with a focus on the prevention and management of Venous Thromboembolism and its complications.
  • To advance the theory and practice of advanced nursing practice through demonstrating expert practice, autonomy in practice clinical leadership, research, and audit.
  • To work with the healthcare team contributing to meeting health service quality/patient safety and resource needs at local and national level and within a patient-centred philosophy.

The advanced practice service is provided by nurses who practice at a higher level of capability as independent, autonomous and expert advanced practitioners. The overall purpose of the service is to provide safe, timely, evidenced based nurse-led care to patients at an advanced nursing level. This involves undertaking and documenting complete episodes of patient care, which includes comprehensively assessing, diagnosing, planning, treating and discharging patients in accordance with collaboratively agreed local policies, procedures, protocols and guidelines and/or service level agreements/ memoranda of understanding.The VTE RANP demonstrates advanced clinical and theoretical knowledge, critical thinking, clinical leadership and complex decision-making abilities.The VTE RANP practices in accordance with the Code of Professional Conduct and Ethics for Registered Nurses and Registered Midwives (NMBI 2014), the Scope of Nursing and Midwifery Practice Framework (NMBI 2015), Advanced Practice (Nursing) Standards and Requirements (NMBI 2017), and the Values for Nurses and Midwives in Ireland (Department of Health 2016).The VTE RANP service provides clinical leadership and professional scholarship in the delivery of optimal nursing services and informs the development of evidence based health policy at local, regional and national levels.The VTE RANP contributes to nursing research that shapes and advances nursing practice, education and health care policy at local, national and international levels.Informal EnquiriesMs. Mairéad Cowan, Director of Nursing – Cancer Services Directorate & Interventional Radiology,Email: mairead.cowan@hse.ie Tel: 061 482386
